What is Family Practice?
Family practice, by definition, is comprehensive, continuing medical care
provided to a family unit by a physician. To be certified as a specialist in
a family practice, a doctor must be trained in the following areas of
medicine: pediatrics, internal medicine, surgery, obstetrics and gynecology,
psychiatry, and community and preventive medicine. In order to be certified
by the American Board of Family Practice, a physician must complete a
three-year family practice residency training program after completion of
medical school.
He or she must also pass a written examination administered by the American
Board of Family Practice and must be recertified every 7 years.

Office Hours and Appointments
Office visits are by appointment only. Please let the receptionist know
the nature of your problem, so that the appropriate time will be scheduled
for
your visit. If, for any reason, you are unable to keep an appointment,
please notify the office 24 hours in advance.

When you need
to reach us after hours for an emergency, please call 407-352-9717 and
the answering
service will contact us immediately.
Cancellation Policy
A $25 fee will be added to your account for any missed (No Show)
appointment. Regretfully, we had to implement this policy in order to give
other patients the opportunity to be cared for in a timely manner. This will
also ensure that our providers’ times are efficiently utilized. We therefore
request that you call 24-hours prior to your appointed time either to cancel
and/or reschedule your appointment.
Telephone Calls and Prescriptions
Our telephone policy is designed to allow us to care for patients with a
minimum amount of interruptions. Calls that are less urgent may be returned
at a later time when patients are not being seen.
In order to ensure accuracy and efficiency, all prescription refills
should be called in by you to your pharmacy directly. Your pharmacy
will then call
our office for approval. Please call in any refill requests at least
one week before you run out of medication. No antibiotics, pain medications,
tranquilizers, or controlled drugs will be refilled without the patient
being seen.
We certainly hope you understand this and assist us with your cooperation.
Fees, Payments, and Insurance
At the end of each office visit, an itemized fee slip will be given
to you except for some selected HMO insurances. This slip contains procedures,
testings, and charges rendered to you at the time of service. Please
take a
moment to review this slip prior to leaving our office.
In an effort to keep our cost down it is customary that payment be made
at the time of service, unless other arrangements are made in advance.
Please note that we do participate and accept assignment with most insurance
companies and we will try to collect your portion based on the amount
allowed by your insurance, such as co-pays, deductibles, and/or any portion.
As a courtesy to you, we will file your claim with your insurance carrier
except for some selected HMO, PPO, and out-of-state companies. However,
all
claims not paid within 30 days will be transferred to your responsibility.
Thus, we do suggest that you familiarize yourself with your policy and
bring
in any required form(s) on every visit if needed.
Please notify us of any changes in your insurance coverage as soon as
possible. Having the most current information is vital in preventing
unnecessary delays in the billing process.
