Referrals to a Specialist You Choose
As a family practice group, we believe that it is our responsibility to assist and guide patients to appropriate specialty care when required. We work closely with patients in selecting specialists, making choices based on specific medical needs, pre-existing relationships, convenience, insurance requirements, and other personal preferences.
To reach our Referral Coordinator call (407) 352-9717 x 105 (voice mail only) or click here to email your request. You will be called back only if there is a problem. At the voice mail prompt please leave the following information:
- Name
- Date of Birth
- Telephone Number
- Primary Care Physician
- Specialist Name
- Date of the Appointment
- Reason for Visit
- Updated Insurance Information
Your referral will be delayed if:
- Your request was not generated from a recent visit. In this event, your request would have to be forwarded to your provider for approval.

You receive a written referral form from another specialist during a visit (For HMO's needing authorization number Only) and it is not dropped off at the front desk or not called in to referral coordinator. All written referrals need to be forwarded to the referral coordinator for processing.
All referrals are subject to the restrictions/conditions imposed by the insurance company. Once your referral is complete, it will be forwarded to the specialist. Please note that depending on the insurance company, it may take up to 10 days for a referral to be fully processed and forwarded to the specialist's office.
Many insurance companies must approve our request for a referral, assign a referral authorization number and forward that information to the specialist. We know this process is sometimes very frustrating but our staff is very conscientious in processing referrals as expeditiously as possible. Please understand that your appointment with the specialist may be denied and you may have to reschedule your appointment if these guidelines are not followed.
